12.5 million US children are obese, but we can make changes to ensure our children live active, healthy lives. Childhood obesity is a major public health problem. In the US, 17% of children are obese, and certain groups of children are even more severely affected by this problem. Nearly 27% of Mexican-American boys are obese, and nearly 30% of non-Hispanic black girls are obese.

There is no single or simple solution to childhood obesity. It is influenced by many different factors, including a lack of access to healthy food and drinks as well as limited opportunities for physical activity in the places where children live, play and learn. Working together, states, communities, and parents can help make the healthy choice the easy choice for children and adolescents.

September is National Childhood Obesity Awareness Month (NCOAM), and you can also pledge to spread the word:

VIRGINIA’S PRE-EXISTING CONDITION INSURANCE PLAN

Eligible residents of Virginia can apply for health insurance coverage through the Pre-Existing Condition Insurance Plan (PCIP) program run by the U.S. Department of Health and Human Services.
Congress created the temporary PCIP program as part of the Affordable Care Act to help uninsured Americans with a variety of medical conditions get affordable coverage rather than be locked out of the system by insurance companies. In 2014 and beyond, insurers will be prohibited from denying coverage to anyone with a pre-existing condition and new competitive marketplaces called Health Insurance Exchanges will give people the opportunity to shop for the policy that best suits their needs. Millions of Americans also will receive tax credits to help make coverage affordable.
PCIP provides Americans living with such conditions as cancer, diabetes, or heart disease the opportunity to obtain insurance coverage. The program covers a broad range of health benefits and is designed as a bridge for people with pre-existing conditions who cannot obtain health insurance coverage in today’s private insurance market.
To qualify for PCIP coverage:

• You must be a citizen or national of the U.S. or lawfully present in the U.S.

• You must have been uninsured for at least the last six months before you apply.

• You must have a pre-existing condition or have been denied coverage because of your health condition. Starting July 1, 2011, you can simply provide a letter from a doctor, physician assistant, or nurse practitioner dated within the past 12 months stating that you have or, at any time in the past, had a medical condition, disability, or illness. A denial from an insurance company is not needed!
PCIP covers a broad range of health benefits, including primary and specialty care, hospital care, and prescription drugs. All covered benefits are available for you, even if it’s to treat a preexisting condition.

In addition to your monthly premium, you will pay other costs. Before most medical benefits are covered, you must first pay a deductible, which varies by your plan option. (see below). But, there is no deductible for preventive services, which can start right away. You will also pay copayments (e.g. $25 for doctor visits; $4 to $40 for most prescription drugs), and 20% of the costs of any other covered benefits you get. Your out-of-pocket costs cannot be more than $5,950 per year. These costs may be higher, if you go outside the plan’s network.

2011 Standard Plan - The 2011 Standard Plan now has two separate deductibles -- a $2,000 medical deductible and $500 drug deductible. The reduced pharmacy deductible is particularly helpful for people who take one or more maintenance medications.

2011 Extended Plan - The Extended Plan has a $1,000 medical deductible and $250 drug deductible plan. Like the Standard Plan, separating the drug and medical deductibles makes this option more valuable for enrollees with one or more maintenance medications.

Health Savings Account Option - The HSA Option has a $2,500 deductible and is eligible to receive favorable tax treatment by the federal government when used with a Health Savings Account (HSA).

Child-Only Rate -To ensure that children have more affordable access to coverage, HHS has established premiums targeted for covering children under PCIP, creating a child-only rate for PCIP enrollees between 0-18 years of age.

The Minnesota Department of Health is pleased to announce the launch of a new, free, online training. The training was developed as part of an EPA grant and is titled, Reducing Environmental Triggers of Asthma in the Home, or RETA-Home. This training is designed for public health nurses (PHNs) and certified asthma educators (AE-Cs) and takes about 40 minutes to complete. It teaches about the triggers of asthma found in the home and suggests simple, realistic interventions that helps limit or eliminate exposure to these triggers.

The program also features a resource section that can be accessed after the training has been completed.

Moms Matter Gala

My name is Lilly Smith, the Loving Steps Resource Mothers program coordinator at Eastern Virginia Medical School. The Loving Steps program is a federally funded community-based program to lower infant mortality and improve the health of mothers and babies. And one of the steps is to improve the self esteem of these low-income, struggling mothers and mothers-to-be.
For the past five years in May, the Loving Steps program celebrated motherhood by inviting the families to what we call the, "Moms Matter Gala". Last year we partnered with members of the community to provide each woman attending who desired one with an after five/semi-formal outfit. This project was such a success, it was decided that it would be worth repeating.
I am requesting your help to make this possible by donating your gently used semi-formal/after five dresses. The event is scheduled for May 24, 2011. Please help us put smiles on pretty faces, and show your support. We are requesting the dresses be turned in by May 16, 2011. I wanted to let you know that CDC will be releasing a Vital Signs focused on asthma this Tuesday, May 3rd (also World Asthma Day).
CDC Vital Signs offers recent data and calls to action for important public health issues (http://www.cdc.gov/vitalsigns/). This program is a call to action each month concerning a single, important public health topic. CDC believes that by focusing on a single topic using multiple media devices, the states might better identify these health problems in their area and work towards their improvement.
Here’s what CDC will do:
1. Hold a media teleconference at noon on May 3;

2. Release an asthma MMWR Early Release May 3;

3. Publish a professionally designed Fact Sheet for consumer audiences, a dedicated website that mirrors the Fact Sheet on the topic;

4. Issue a media release;

5. Send out a series of announcements via social media tools (Twitter,Facebook, etc.);

6. Send out a Capitol Hill alert through CDC-Washington Office.

Coalition Perspectives: Should we collect BMI in schools?

There has been a lot of debate around whether or not schools should collect Body Mass Index (BMI) measurements on school children. There is additional debate also on whether or not a parent should be notified of the results.

It is important to note that just because BMI is collected often in the schools that children are not overweight simply because of what goes on at the school. Children live in homes and communities with many factors contributing to their activity levels, nutrition intake, and weight. We simply measure children in schools because that is where the normative population of children reside. If you want to go bird watching, you go to the forest - if you want to measure children, you go to the schools.

The challenge with BMI is that from a community and regional perspective we ALL need this information to be collected. It certainly helps the school systems to make wise decisions about resources based on what is working (or not working) to keep kids healthy in schools. It also helps community decisions on where to focus efforts, what factors are helping or hurting children's health, and to share best practices across communities. More importantly, this information helps us, as a region, to leverage resources and apply for large scale funding to fight childhood obesity.

Certainly, there continues to be debate on how to collect BMI in a manner that is sensitive and appropriate, as well as how or if to notify parents. We don't have these same debates about vision screening or notifying parents if their child has lice or is behind on immunizations. As a society we need to move forward in considering weight and BMI as medical information, just like other screenings. Certainly any screening for any condition may not be accurate - which is why a referral to a physician is the best way to make sure the child gets an appropriate diagnosis and treatment for whatever the issue may be. We need to move towards helping our community understand the importance of BMI as a medical indicator. While body type may impact results - it is an accurate measure more often than it is not. And, from a population health perspective, it's in an important, easily collected, accurate measurement.

CINCH continues to support the collection of BMI data through our schools, in physician offices, as an important measure of our community's health.

Exceptional verbal, written, interpersonal and organizational skills necessary. Demonstrated proficiency in MS Office Suite. Previous experience in course curriculum development, knowledge of child development and effective parenting techniques preferred. Familiarity with community and community support agencies. Must be flexible and adaptable to the meet the needs of the families and program. Must possess a valid Virginia driver’s license and be able to meet the insurance requirements of the hospital, if required by position. “Co-Parenting: In the Child’s Best Interest” workshop

The impact of the availability of school vending machines on eating behavior during lunch: the Youth Physical Activity and Nutrition Survey.

Abstract
Childhood obesity is a major public health concern and is associated with substantial morbidities. Access to less-healthy foods might facilitate dietary behaviors that contribute to obesity. However, less-healthy foods are usually available in school vending machines. This cross-sectional study examined the prevalence of students buying snacks or beverages from school vending machines instead of buying school lunch and predictors of this behavior. Analyses were based on the 2003 Florida Youth Physical Activity and Nutrition Survey using a representative sample of 4,322 students in grades six through eight in 73 Florida public middle schools. Analyses included χ2 tests and logistic regression. The outcome measure was buying a snack or beverage from vending machines 2 or more days during the previous 5 days instead of buying lunch. The survey response rate was 72%. Eighteen percent of respondents reported purchasing a snack or beverage from a vending machine 2 or more days during the previous 5 school days instead of buying school lunch. Although healthier options were available, the most commonly purchased vending machine items were chips, pretzels/crackers, candy bars, soda, and sport drinks. More students chose snacks or beverages instead of lunch in schools where beverage vending machines were also available than did students in schools where beverage vending machines were unavailable: 19% and 7%, respectively (P≤0.05). The strongest risk factor for buying snacks or beverages from vending machines instead of buying school lunch was availability of beverage vending machines in schools (adjusted odds ratio=3.5; 95% confidence interval, 2.2 to 5.7). Other statistically significant risk factors were smoking, non-Hispanic black race/ethnicity, Hispanic ethnicity, and older age. Although healthier choices were available, the most common choices were the less-healthy foods. Schools should consider developing policies to reduce the availability of less-healthy choices in vending machines and to reduce access to beverage vending machines. 2010 American Dietetic Association, Dr. Welch

They are not FDA controlled/monitored therefore the quality of the product and even the ingredients of the cigarette are not guaranteed.
The amount of nicotine varies.
Other chemicals were found in the product as well, such as Nitrosamine (a carcinogen) and diethylene glycol (a toxic chemical used in antifreeze).
Minors may purchase the product – many are sold online.
The FDA has taken action against several manufactures for false advertisements and poor manufacturing practices.

We need to ensure to notify our youth that they are not safe.

Today, the National Academy for State Health Policy (NASHP) and the Maximizing Enrollment for Kids program announced support of U.S. Department of Health and Human Services Secretary Kathleen Sebelius' goal to enroll the five million children who are uninsured, but eligible for Medicaid or the Children's Health Insurance Program (CHIP). NASHP is the National Program Office for the Robert Wood Johnson Foundation's Maximizing Enrollment for Kids. Coinciding with the HHS challenge announcement, NASHP also released its State CHIP fact sheets http://nashp.org/node/2089 . The fact sheets, developed from data collected in a 2008 NASHP survey and supported by the David and Lucile Packard Foundation, provide a profile of program characteristics, including adoption of key enrollment strategies.

As a fellow child health professional, I wanted to let you know about our new nonprofit Boost 'em in the Back Seat website at http://www.boosterseats4safety.org. You may already be familiar with the 6-minute Boost 'em in the Back Seat video, which is available for free and easy download (or streaming, if you prefer) on the site. The video includes high-threat content that evokes a sense of vulnerability to motor vehicle hazards, as well as high-efficacy content that provides caregivers with the confidence and knowledge to protect one's family from motor vehicle risks. Evaluation research has shown the video-intervention to significantly increase child passenger safety knowledge, risk-reduction attitudes, perceptions of fear and efficacy, and observed booster seat use. Also on the website are free 30-second PSAs, downloadable resources, and parent education pages, all of which are available for use in your work to keep children safe.

The Department of Medical Assistance Services (DMAS) will be conducting a special WebEx training session for the Virginia Perinatal Council members on the basics of public health insurance for pregnant women and newborns. DMAS staff will cover basics on eligibility for pregnant women and newborns, the application process and any special rules for pregnant women and newborns. DMAS staff will cover tips that the Perinatal Council members may chose to take action on to help individuals expedite their application process. There will also be time for questions and answers at the end of the session.
The WebEx is open to all Perinatal Council members. There are slots for 50 registrants (across the state), so if there are those who will be viewing together from one computer, it would be helpful for only one to register (as to only take one registration slot).

Our playgroups feature open learning centers that include housekeeping, train tables, dressup closets, book and puzzle corners, toy cars, educational activities, and arts and crafts. Circle time activities include children’s songs, music with instruments, finger plays, stories, field trips and outside playtime on age-appropriate playground equipment. Family Focus also provides on-site professional parent educators, parent education classes and special activities for families.

The goal of this program is to offer effective strategies to teach young children how to get along with others, solve problems, use self control, manage anger and make healthy choices. In addition to the Al's Pals curriculum we will add a healthy nutrition and fitness component from the Color Me Healthy curriculum.

New Centering Pregnancy site on the Peninsula ready to schedule patients

Beginning in early September, Riverside Family Medicine Residency will be offering Centering Pregnancy to its maternity patients. Centering Pregnancy is a new model for prenatal care. Prenatal care occurs in a group setting with 8 to 12 pregnant women and two facilitators. Physical assessment (fetal heart tones, blood pressure, weight checks) is followed by education and support. Randomized trials have shown that this unique model of prenatal care reduces the rate of preterm birth in women who participate.Women can be scheduled now for an intake appointment and for enrollment in the first group. Please encourage pregnant women who would benefit from Centering Pregnancy to phone 594-3800 and ask for Michelle Wells, RN. The first group will meet in the afternoon on Wednesday, September 8 and is for women with due dates in February and March. The next group will begin in late October for women due in March and April. WHRO, the Future of Hampton Roads, the Hampton Roads Partnership, and The Planning Council will once again partner with the HRCCE to engage participants in considering the role of citizens in the hard policy and budget decisions that affect their lives.
Two nationally-known speakers with deep ties to Hampton Roads will set the context for interactive sessions. Bob O'Neill was widely recognized for his "reinvention" of the government of Hampton as city manager from 1984 to 1997. He is currently executive director of the International City/County Management Association (ICMA), a membership organization dedicated to creating excellence in local government through professional management.
Bill Schneider was born in Portsmouth and attended Wilson High School. He is one of the country's foremost political analysts, having been CNN's senior political analyst from 1990 to 2009. Bill is Hirst Professor of Public Policy at George Mason University and Distinguished Senior Fellow and Resident Scholar at Third Way, a Washington think tank. He is also a contributing editor to National Journal and The Atlantic Monthly. Bill has been labeled "the Aristotle of American politics" by The Boston Globe.
The first summit in April 2009 included small-group dialogues which focused on how to improve the way citizens communicate and engage with their elected officials in Hampton Roads. The HRCCE Information and Communication Team grew out of those discussions and the team has continued to meet over the past year. On April 10th, team members will present their recommendations for how to improve public-decision-making for all parties.
In the words of Thomas Jefferson, “The government is the strongest of which every man feels himself a part.” Please join us as we bring together citizens serving as elected or appointed officials and other members of the public to build relationships, increase mutual understanding and strengthen public decision making for the future of our democracy.

The American Lung Association (ALA) of DC, DC Control Asthma Now (DC CAN), the Maryland Asthma Control Program (MACP) and the Virginia Asthma Control Project (VACP) have partnered to address the burden of asthma within the National Capital region. This partnership provides a unified effort in addressing asthma in numerous jurisdictions, in which geographical boundaries are shared. Joint surveillance and programmatic activities by DC CAN, MACP and VACP will allow for the greatest reach to those affected by asthma where they live, work and play.
To further this partnership, ALA of DC, DC CAN, MACP and VACP will jointly sponsor a one day conference to address many of these issues. This regional asthma conference during Asthma Awareness Month in May 2010 will enable regional asthma stakeholders to come together and establish priorities for reducing the burden of asthma within the National Capital region. The conference will provide an opportunity for stakeholders to understand the issues associated with asthma in Washington, DC, Maryland and Virginia, while brainstorming how efforts can jointly create a solution. Award-winning schools are recognized with a framed certificate and award banner from USDA to showcase their achievement in the community. HUSSC schools also receive national recognition by being listed on the Team Nutrition Web site. Awards are certified for 4 years from the date of approval.

NRTC - 12 week course

The National Respiratory Training Center is sponsoring its 12 week distance learning Asthma Assessment and Management Course which will begin May 7, 2010 and ends July 30, 2010 with study days in Norfolk, VA..

The course covers (1) Asthma facts (2) What is Asthma and who gets it (3) Physiology of the respiratory tract (4) The mechanisms of Asthma (5) Initial assessment and diagnosis of Asthma and classification of severity (6) Controlling the things that make Asthma worse (7) Asthma medications and devices (8) Managing Asthma over time (9) Management of special populations and special situations (10) Patient education to promote self-management and (11) Spirometry.

Continuing education credits are provided for Nurse Practitioners (24.8 + 13.9 pharmacology AANP) Physician Assistants (11.5 AAPA), Respiratory Therapists (22.5 AARC) and Nurses (49.8 CA BRN - applicable nationally) and recently added Pediatric Nurse Practitioners (24.8 + 13.9 pharmacology NAPNAP)

Weeks 1-6 are for home study with the materials that are mailed to each student. During week 6 there are 2 on-site course study days on June 18-19, 2010, at the Eastern VA Medical School, Dept of Pediatrics, Norfolk, VA and will be taught by our NRTC certified trainer Karen Rance. Weeks 7 - 12 are for more home study and to complete final projects.

Scholarships have been secured and the cost of the course is $150. The cost without scholarships would be $595.

Graduates are prepared to provide excellent patient care. Many course graduates choose to become nationally certified by taking the National Asthma Educator Certification Exam. The course also includes sample AE-C type questions.

Cost of Health Care: A Pediatric Case Study: Bryan R. Fine, MD, MPH
Dr. Fine earned his Medical Doctorate at the University of South Florida in Tampa, and completed his Masters in Public Health at George Washington University where his interests were health policy and management. He completed his residency in pediatrics at the Children's National Medical Center in Washington, DC, after which he served one year as a project manager for a computerized physician order entry implementation. He has given health policy Grand Rounds, written a national column on Pediatric Health Policy, and is the founder of the newly-created Virginia Child Health Policy Center. Currently, he is Director of the Division of Pediatric Hospital Medicine at Children's Hospital of The King's Daughters.
